just click
on the link above, then print the page, fill it out and mail or bring to

(Word) document is a one that has been created in Microsoft Word.
In order to view the document you must have Microsoft Word installed on
your machine. Left click
on the above link to open the file in word, print it, fill out the
information and mail or drop it off at Recckio's. To download the
file and later print it, right click
on the file and save it on your hard drive. Go back to the file,
open it, print it and fill it out the information. Email the
document to

or drop it of at Recckio's.
